Start of Care (SOC) RN - Home Health

As a Start of Care RN, you'll serve patients across Aurora to Castle Rock, ensuring smooth transitions, fostering strong relationships with families, and providing expert clinical care. Key Responsibilities:
- Patient Assessment & Onboarding: Conduct initial assessments, onboard new caregivers, and collect necessary documentation.
- Clinical Care: Coordinate care with PCPs, manage a small caseload, and provide re-certification coverage as needed.
- Documentation: Complete accurate charting within 48 hours and prevent billing issues.
- Quality Assurance: Maintain high QA standards and ensure positive patient feedback.
- Time Management: Meet productivity goals, including one admin day weekly. Reliable transportation is required.
**Familiarity with the client’s Patient Assessment Tool (PAT) scoring is beneficial.**

Qualifications
Education:
- BSN required.
Licensure:
- Active RN license in Colorado.
Experience:
- 3-5 years of nursing experience.
- Pediatric home health experience preferred.
- OASIS knowledge is a plus.
