Key Responsibilities:

Patient Onboarding & Assessment:
- Conduct initial assessments for home health services.
- Welcome and orient new caregivers and families.

Care Coordination:
- Collaborate with PCPs to develop care plans.
- Support a small caseload and cover recertifications.

Documentation:
- Complete accurate charting within 48 hours.
- Monitor patient risks and escalate as needed.

Quality Assurance:
- Achieve high QA rates and minimize billing issues.
- Gather positive feedback from families.

Time Management:
- Manage time effectively with a dedicated admin day.
- Ensure reliable transportation for commuting.

Job Requirements

Education: BSN preferred.

Licensure: Current RN license in Colorado.

Experience:
- 3-5 years of nursing experience.
- Pediatric home health experience a plus.
- OASIS experience is a plus.

Skills:
- Strong time management and bedside manner.
- BLS certification.
- Organized and detail-oriented.
- Experience with care coordination and PCPs.
- Sales or customer service background is a plus.
