Position Overview:
The Start of Care (SOC) RN at the client is responsible for initiating comprehensive patient assessments and ensuring a smooth transition into home health care services. This role requires a nurse with a strong background in pediatric home health, excellent time management skills, and a commitment to high-quality patient care. The SOC RN will be the face of the client to new caregivers and families, ensuring thorough and efficient onboarding and care coordination.
Reports To: Clinical Nurse Manager
Key Responsibilities
Patient Assessment and Onboarding:
- Conduct initial patient assessments to determine eligibility for home health services.
- Engage with new caregivers and families, providing a welcoming and informative introduction to Abby Care.
- Collect and review all necessary documentation in preparation for each Start of Care visit.
Clinical Care and Coordination:
- Coordinate care with Primary Care Physicians (PCPs) and therapy providers to ensure comprehensive care plans.
- Maintain a small caseload, providing ongoing support and coordination for patients within the assigned territory.
- Provide coverage for recertifications for nurses who are out of the office.
Documentation and Compliance:
- Ensure thorough and accurate charting with a high attention to detail.
- Complete charting within 48 hours of patient visits.
- Monitor and evaluate patient qualifiers for risks, escalating concerns to clinical leadership as needed
Quality Assurance:
- Achieve a high Quality Assurance (QA) pass rate.
- Minimize 485 billing issues through diligent documentation and follow-up.
- Collect positive survey results from families, ensuring they feel adequately supported and treated with dignity and respect.
Time Management and Productivity:
- Manage time effectively to meet productivity expectations, including a dedicated administrative day each week for charting and preparation.
- Be flexible with driving and commuting; reliable transportation is essential.

Job Requirements
Qualifications:
- Education: Bachelor of Science in Nursing (BSN).
- Licensure: Current Registered Nurse (RN) license in the state of Colorado.
Experience:
- 3-5 years of nursing experience.
- Previous pediatric home health agency experience.
- OASIS experience preferred for certain roles.
Skills:
- Strong time management skills.
- Excellent bedside manner.
- Basic Life Support (BLS) certification.
- Highly organized and efficient.
- Thorough with charting and attention to detail.
- Comfortable with care coordination and engaging with PCPs and therapy providers.
- Background in sales or customer service is a plus.
- Familiarity with Abby Care’s Patient Assessment Tool (PAT) scoring.
