Job Description
We are currently collaborating with a thriving home health organization in search of a dedicated and caring Registered Nurse (RN) to join their team on a full-time basis. This role is well-suited for an RN who thrives in a role that offers autonomy, case management responsibilities, and the opportunity to forge meaningful connections with patients within a home care environment.
Job Description
The RN will be responsible for delivering expert nursing care to patients residing in Aurora, Central Denver, and the Littleton area.
The role encompasses a variety of tasks, including:
- Commencing patient care visits
- Conducting routine skilled nursing visits
- Handling recertifications
- Managing discharges
- Ongoing case management
The work schedule for this position is Monday through Friday during daytime hours.

- Medication management
- Wound care
- Coordinating with therapy services
- Providing patient education and devising care plans

Requirements
- Active RN license in Colorado
- Previous home health experience preferred
- OASIS documentation experience required
- Familiarity with KanTime EMR strongly preferred
- Strong clinical assessment and case management skills
- Reliable transportation and ability to travel throughout the assigned territory
