Location: North area - Boulder, Superior, Erie, Louisville & surrounding areas

Key Responsibilities:
- Conduct personalized in-home evaluations and assessments.
- Develop individualized treatment plans and maintain accurate medical records.
- Collaborate with physicians, staff, and external services for effective care coordination.
- Monitor patient care quality and ensure treatment plan implementation.
- Review caseload documentation regularly to track progress and treatment effectiveness.

Job Requirements
Requirements:
- Active Colorado RN license
- Minimum of 1 year nursing experience (hospital/ER experience preferred)
- Experience in home health is advantageous
- Proficient with EMR/computer systems
- Knowledge of wound care and IV therapy
- BLS/CPR certification
- Access to reliable transportation
