Job Description
WSi Healthcare is working with a growing healthcare organization that’s redefining family caregiving, and they’re building a Talent Community for future RN Case Manager openings in Colorado!
As an RN Case Manager, you’ll coordinate care plans, oversee and support CNAs, and work closely with physicians, therapists, and families to ensure the best outcomes. This role is ideal for nurses who want to make a direct impact while being part of an innovative, mission-driven team.

Pay range: $80,000–$84,000 annually

What’s needed:

Current RN license (Colorado or compact)

2+ years RN experience (at least 1 year home health experience preferred)

Reliable transportation & flexibility for local travel

Job Requirements
- An Associate's or Bachelor's degree in Nursing.
- Current and valid license as a Registered Nurse in the state of Colorado or a Compact/Multi-state Unencumbered License required.
- Minimum 2 years of experience as a Registered Nurse in a clinical setting (1 year of home health experience preferred).
- Case management/supervisor experience and experience in both pediatric/adult care are preferred.
- Knowledge of the Medicaid paid family caregiving program and CNA scope of practice preferred (training provided if needed).
- Reliable transportation, valid driver's license, and current auto insurance are required.
- Ability to work flexible hours, travel locally, and provide on-call service to patients/families.
- Knowledge of OASIS preferred (training provided if needed).
