RN Clinical Manager

We are in search of a dedicated Clinical Manager to supervise skilled nursing care delivery, manage field staff, and ensure top-notch care for our clients. If you have a strong commitment to leadership, quality, and patient-centered care, we encourage you to consider this crucial role.
Clinical Management Duties:
- Supervise and lead field staff, such as RNs, LPNs, PTs, OTs, STs, MSWs, and CNAs, to ensure high-quality care provision.
- Review and approve field staff documentation, visit time entries, and address any discrepancies.
- Develop and maintain accurate CNA care plans, delegation forms, and supervisory visits.
- Address client concerns by collaborating with leadership to conduct thorough investigations and reach effective resolutions.
- Plan and conduct monthly CNA staff meetings, training sessions, and continuous educational opportunities (12 CEUs annually).
- Conduct competency assessments and annual performance evaluations for CNAs.
- Ensure compliance with Medicare, state regulations, and requirements from third-party payers.
Field Nursing Responsibilities:
- Perform initial and ongoing assessments, including OASIS, to create care plans.
- Deliver skilled nursing care, preventive services, and education to clients and their families.
- Coordinate care with interdisciplinary teams and make referrals to additional services when necessary.
- Conduct essential home health visits and maintain thorough documentation in the EMR system.

Job Requirements
Qualifications:
- Graduate of an accredited school of professional nursing; BSN preferred.
- Active RN license in the state.
- Minimum of 2 years of nursing experience, with at least 1 year in a supervisory role preferred.
- Strong knowledge of federal and state home health care regulations.
- Proficiency in electronic nursing documentation systems.
- Excellent communication, leadership, and interpersonal skills.
