RN Clinical Manager

Job Description

We are seeking a Clinical Manager to provide skilled nursing care, supervise field staff, and ensure exceptional care for our clients. Responsibilities:
Clinical Management Duties:
- Oversee and supervise field staff, including RNs, LPNs, PTs, OTs, STs, MSWs, and CNAs, ensuring high-quality care.
- Review and approve field staff documentation, visit time entries, and resolve discrepancies.
- Develop and maintain accurate CNA care plans, delegation forms, and supervisory visits.
- Manage client complaints, collaborating with leadership to ensure investigations and resolutions.
- Plan and conduct monthly CNA staff meetings, training, and continuing education (12 CEUs annually).
- Perform competency evaluations and annual performance reviews for CNAs.
- Maintain compliance with Medicare, state regulations, and third-party payer requirements.
Field Nursing Duties:
- Conduct initial and ongoing assessments, including OASIS, to establish plans of care.
- Provide skilled nursing care, preventative services, and teaching for clients and families.
- Coordinate care among interdisciplinary teams and refer clients to additional services as needed.
- Perform home health visits as required, documenting thoroughly in the EMR system.

Job Requirements
Qualifications:
- Graduate of an accredited school of professional nursing; BSN preferred.
- Active RN license in the state.
- Minimum of 2 years of nursing experience, with at least 1 year in a supervisory role preferred.
- Strong knowledge of federal and state home health care regulations.
- Proficiency in electronic nursing documentation systems.
- Excellent communication, leadership, and interpersonal skills.
