Position Summary:
We are seeking a dedicated and skilled RN Case Manager to join our Home Health team in Pueblo. This role provides comprehensive nursing care in accordance with each client’s individualized plan of treatment, including health and psychosocial evaluations, monitoring services, patient/family education, and direct nursing care. The RN Case Manager also supervises LPNs, Home Health Aides, and support staff.

Key Responsibilities:

Conduct initial and ongoing comprehensive patient assessments (including OASIS).

Develop, initiate, and revise individualized plans of care.

Educate and counsel patients and families on disease processes, self-care, and prevention strategies.

Deliver specialized nursing services and initiate preventive/rehabilitative procedures.

Supervise and provide guidance to LPNs and Home Health Aides.

Maintain accurate clinical documentation and reports required by Medicare and other payers.

Communicate patient status and needs with physicians and interdisciplinary teams.

Coordinate patient care and make appropriate referrals.

Plan and facilitate patient discharge.
-
Participate in ongoing professional development and complete required CEUs.

Be available to patients and staff Monday–Friday, 8 a.m.–5 p.m., and participate in on-call rotation as needed.

Manage visit caseloads according to a structured point system (start of care, resumption, discharge, skilled visits, supervisory visits, etc.).

Cover visits outside the usual caseload/geographic area as needed.

Job Requirements

What You’ll Need to Join Our Team:
-
Associate’s or Bachelor’s degree in Nursing from an accredited program.
-
Active RN license in Colorado (required).
-
Minimum of one year of home health experience (preferred).
-
Current driver’s license, automobile insurance, and reliable transportation.

BLS Certification through the American Heart Association
-
Current vaccinations
-
Ability to pass a background check and drug screen
