RN Case Manager – Home Health (Hybrid)
Location: Longmont, CO
We’re hiring a Full-Time RN Case Manager for a home health client in the Longmont area. This role is hybrid, with approximately 60% in-person patient visits and 40% remote work.
What You’ll Do:

Manage a patient caseload and ensure quality, timely care

Oversee and support Home Health Aides/HHAs
-
Complete patient assessments and develop care plans

Coordinate care with PCPs, therapy providers, and internal teams

Educate patients and families on care needs
-
Maintain accurate electronic documentation (24-hour turnaround)
-
Participate in on-call rotation (about one weekend/month), holiday rotation, and occasional weekday phone triage
Schedule:

Hybrid: 60% in-person, 40% remote

Local travel required
Compensation & Benefits:

$80,000–$84,000 annual salary + bonus potential

Job Requirements
Requirements:
-
Active Colorado RN license or Compact RN license
-
Associate's or Bachelor’s Degree in Nursing
-
2+ years of clinical RN experience (home health preferred)
-
Case management or supervisory experience a plus
-
Reliable transportation, driver’s license, and auto insurance
-
Knowledge of OASIS and Medicaid family caregiving programs preferred (training available)
