Pediatric Home Health IHSS Case Manager - RN
Job Description
Attention RNs with IHSS Home Health Experience!
WSi is seeking dedicated and compassionate Registered Nurses with at least 3 years of nursing experience and IHSS experience at a home health care provider. If you're passionate about transforming care and working directly with families, this is the opportunity for you!
In this hybrid role, you’ll spend at least 4 days per week providing care directly in the home, while using the remaining time to coordinate care remotely.
Key Responsibilities:
- Develop and implement care plans for home health patients.
- Collaborate with families to provide health updates and coaching support.
- Leverage digital health technology to maximize impact and communicate with your care team.
- Work cross-functionally with other teams to ensure the best health outcomes.

Job Requirements
- Must be a registered nurse with at least 3+ years of nursing experience with IHSS experience at a home health or home care provider
- At least 1 year of home health experience
- Licensed in the state of Colorado
- Must currently reside in the Denver area
- Must have BLS certification
- Strong communication and documentation skills
