RN Case Manager Overview:

As an RN Case Manager, you’ll lead in coordinating and delivering high-quality, compassionate care in patients' homes. This role is perfect for a detail-oriented nurse with strong communication skills and a passion for patient-centered care. Collaborate with families, physicians, and internal teams to deliver impactful, tailored care plans that drive exceptional outcomes.

Key Responsibilities:

Job Requirements

Qualifications:

  • Education: Minimum requirement is an Associate Degree in Nursing (ADN); Bachelor of Science in Nursing (BSN) is preferred.
  • Licensure: Valid Registered Nurse (RN) license in the state of Colorado.

Experience:

  • Minimum of one year of nursing experience.
  • Preferred experience in pediatric care.
  • Experience in home health care (adult or pediatric) is advantageous.

Skills:

  • Exceptional organizational, efficiency, and independence skills.
  • Flexibility in scheduling and ability to adapt to change.
  • Excellent relationship-building and communication skills.
  • Reliable transportation is necessary.
  • Comprehensive understanding of the CNA scope of practice (training available if required).
