Reports To: Clinical Lead

Key Responsibilities:

Patient Care & Coordination:

• Manage a diverse caseload with a focus on quality care and efficient coordination.
• Coach and support CNAs to maintain high standards and adherence to clinical guidelines.
• Create and implement individualized care plans based on patient assessments.
• Collaborate with PCPs, therapy providers, and support teams for comprehensive care.
• Educate and support patients and families about their healthcare needs.

Clinical Documentation:

• Maintain accurate, timely electronic charting, preferring real-time updates.
• Ensure documentation meets regulatory standards with a 24-hour turnaround.

Quality Assurance:

• Deliver compassionate care to ensure caregiver satisfaction.
• Monitor and improve patient readmission rates.
• Manage extended visits and complete administrative tasks efficiently.

Qualifications:

• Education: BSN preferred.

• Licensure: Valid RN license in Colorado.

• Experience:

  • At least one year of nursing experience.
  • Pediatric care experience preferred.
  • Home health care experience is a plus.

• Skills:

  • Strong organizational and independent work skills.
  • Flexible scheduling and adaptability.
  • Excellent communication and relationship-building.
  • Reliable transportation needed.
  • Understanding of CNA scope of practice (training available).
