Responsibilities

Provide comprehensive, patient-focused nursing care in the home setting.

Perform assessments, manage care plans, and monitor patient progress.

Administer medications, wound care, and skilled nursing services based on patient needs.

Educate patients and families on treatment plans, safety, and ongoing care.

Collaborate with physicians, therapists, and other interdisciplinary team members.

Maintain accurate, timely documentation according to agency and regulatory standards.

Support continuity of care through clear communication and thoughtful clinical judgment.

Active RN license in the state of Colorado.
