We’re seeking a dedicated RN Case Manager to provide skilled nursing care, coordinate services, and guide patients through their care journey in the comfort of their homes.

What You’ll Do:

  • Perform initial and ongoing patient assessments, including OASIS

  • Develop, initiate, and revise individualized care plans

  • Educate patients and families on disease management and self-care techniques

  • Provide hands-on skilled nursing services

  • Supervise and guide LPNs and HHAs

  • Maintain clear and accurate documentation in line with Medicare and agency standards

  • Communicate changes in patient condition to physicians and interdisciplinary team

  • Plan discharges and manage referrals to other services as needed

  • Participate in ongoing training and complete a minimum of 12 CEUs per year

  • Be available Monday–Friday (8 a.m. – 5 p.m.) with potential on-call rotation


Visit Structure & Productivity Expectations:

  • Case Managers work on a point-based system:

    • Start of Care = 2 pts, Resumption/Discharge/Recert = 1.5 pts, Skilled Visits = 1 pt, Supervisory/Brief Visits = 0.5 pts

  • Expected weekly total: 25–30 points

  • Additional visits beyond 30 points will be compensated per visit

  • Low-census weeks may require office-based work or support in other geographic areas

  • Actively contribute to a positive work environment that prioritizes patient well-being.

Job Requirements

  • What We’re Looking For:

    • Current RN license (Colorado) in good standing

    • At least 1 year of home health or related nursing experience preferred

    • Knowledge of OASIS, care planning, and Medicare documentation

    • Strong communication and clinical assessment skills

    • Ability to work independently and as part of a team

    • Reliable transportation, valid driver’s license, and auto insurance
