We’re seeking a dedicated RN Case Manager to provide skilled nursing care, coordinate services, and guide patients through their care journey in the comfort of their homes.
What You’ll Do:

Perform initial and ongoing patient assessments, including OASIS
-
Develop, initiate, and revise individualized care plans
-
Educate patients and families on disease management and self-care techniques
-
Provide hands-on skilled nursing services
-
Supervise and guide LPNs and HHAs
-
Maintain clear and accurate documentation in line with Medicare and agency standards
-
Communicate changes in patient condition to physicians and interdisciplinary team
-
Plan discharges and manage referrals to other services as needed
-
Participate in ongoing training and complete a minimum of 12 CEUs per year
-
Be available Monday–Friday (8 a.m. – 5 p.m.) with potential on-call rotation
Visit Structure & Productivity Expectations:

Case Managers work on a point-based system:
-
Start of Care = 2 pts, Resumption/Discharge/Recert = 1.5 pts, Skilled Visits = 1 pt, Supervisory/Brief Visits = 0.5 pts

Expected weekly total: 25–30 points
-
Additional visits beyond 30 points will be compensated per visit
-
Low-census weeks may require office-based work or support in other geographic areas
- Actively contribute to a positive work environment that prioritizes patient well-being.

Job Requirements

What We’re Looking For:
-
Current RN license (Colorado) in good standing
-
At least 1 year of home health or related nursing experience preferred
-
Knowledge of OASIS, care planning, and Medicare documentation
-
Strong communication and clinical assessment skills
-
Ability to work independently and as part of a team
-
Reliable transportation, valid driver’s license, and auto insurance
