We’re seeking a dedicated and compassionate Hospice RN Case Manager to provide high-quality, patient-centered care in Grand Junction, CO. This role allows you to make a meaningful impact while working in a supportive and fulfilling environment.
Coverage Area: Travel throughout the territory required, primarily Mesa County
Schedule: Monday–Friday | 8:00 AM – 5:00 PM

As an RN Case Manager, You Will:

Assess and monitor patients’ physical, emotional, and psychological needs
-
Develop hospice care plans aligned with patient wishes and goals

Provide direct nursing care, including medications, treatments, and interventions

Manage pain and symptoms effectively

Educate and support patients’ families and caregivers
-
Collaborate with an interdisciplinary care team

Maintain accurate and timely documentation
-
Participate in on-call rotation as required

Job Requirements

Qualifications
-
Graduate of an accredited nursing program
-
Current RN license in the state of operation
-
Minimum 1 year of nursing experience (3+ years preferred)

Hospice or related experience preferred
-
Knowledge of hospice philosophy of care

Strong clinical and documentation skills
-
Valid driver’s license and reliable, insured vehicle
