Registered Nurse (Case Management) - Contract
Job Description
I’m currently partnering with a well-known university health system to help fill a temporary RN Case Management role! This is a great opportunity for an experienced, team-oriented nurse who enjoys patient coordination and community-based care.
What You’ll Be Doing:

Supporting patients through care coordination and follow-ups
-
Helping families connect with specialty and community resources
-
Providing clinical support and patient education
-
Triaging patient calls and collaborating with a multidisciplinary care team
About the Clinics:
You’ll be splitting time between two established primary care clinics, one in Southwest Denver and one in Aurora, serving diverse communities in a collaborative, patient-centered environment.
Schedule: Full-time, Monday–Friday, day shift
Location: Denver & Aurora, CO
Type: Temporary - 11/10/2025 - 2/10/2026

Job Requirements
What We’re Looking For:

Current RN license (or ability to obtain Colorado license before start)

Bachelor’s in Nursing

1+ year of RN experience preferred

Bilingual (Spanish/English) is a plus
