WSi Healthcare is seeking a compassionate and experienced Registered Nurse (RN) Case Manager for a full time home health position in the Colorado Springs area. This hybrid opportunity allows you to make a meaningful impact by coordinating and delivering exceptional in home care to patients and their families.

Responsibilities

Manage a diverse caseload and oversee individualized patient care plans.

Coordinate care with physicians, therapists, and support teams to ensure comprehensive patient outcomes.

Supervise and guide Home Health Aides (HHAs) to maintain clinical quality standards.

Conduct patient assessments, provide education, and support patients and families through the care process.

Maintain accurate and timely documentation in compliance with regulatory and agency standards.

Participate in an on call rotation (approximately one weekend per month).

Requirements

Active Registered Nurse (RN) license in the state of Colorado.

2+ years of nursing experience; home health experience strongly preferred.

Case management or supervisory background a plus.

Knowledge of OASIS preferred (training available).

Reliable transportation, valid driver’s license, and current auto insurance required.

Strong communication, organization, and problem solving skills.
