Location: North area – Boulder, Superior, Erie, Louisville & surrounding areas

Key Responsibilities:

Conduct personalized in-home evaluations and assessments.

Develop individualized treatment plans and maintain accurate medical records.

Collaborate with physicians, staff, and external services for effective care coordination.

Monitor patient care quality and ensure treatment plan implementation.

Review caseload documentation regularly to track progress and treatment effectiveness.

Job Requirements
Qualifications:

Current Colorado RN license
-
Minimum 1 year of nursing experience (hospital/ER experience preferred)

Home health experience is a plus
-
Proficiency with EMR/computer systems

Experience with wound care and IV therapy
-
BLS/CPR certification
-
Reliable transportation
