Registered Nurse - Pediatric Case Manager

Position Overview:
We’re seeking a skilled RN Case Manager to oversee comprehensive patient care and coordination. This role requires strong organization, communication, and a passion for high-quality care. You'll work closely with patients, families, PCPs, therapy providers, and internal teams to drive optimal outcomes.
Hybrid Schedule:
- 60% in-person (minimal in-office time for meetings/training)
- 40% remote
Key Responsibilities:
Patient Care & Coordination
- Manage a diverse caseload with a focus on quality care and efficiency.
- Provide oversight, coaching, and support to CNAs.
- Develop and implement personalized care plans based on patient goals.
- Coordinate with PCPs, therapy providers, and support teams for seamless care.
- Educate and support patients and families on treatment and health management.
Clinical Documentation
- Maintain accurate, real-time electronic charting.
- Ensure compliance with regulatory and client standards (24-hour turnaround).
Communication & Collaboration
- Build strong relationships with PCPs, therapy providers, and internal teams.
- Escalate concerns to Clinical Leads as needed.
- Participate in on-call rotation (~one weekend per month), holiday rotation, and possible weeknight phone triage.

Job Requirements
Qualifications:
- Associate or Bachelor’s Degree in Nursing
- Active RN license in Colorado or a Compact/Multi-state Unencumbered License
- Minimum 2 years of clinical RN experience (home health experience preferred)
- Case management/supervisory and pediatric/adult care experience a plus
- Familiarity with Medicaid paid family caregiving & CNA scope of practice (training available)
- Reliable transportation, valid driver’s license, and auto insurance
- Flexible schedule with local travel and on-call availability
- OASIS knowledge preferred (training provided)
