Position Overview:

We’re seeking a skilled RN Case Manager to oversee comprehensive patient care and coordination. This role requires strong organization, communication, and a passion for high-quality care. You'll work closely with patients, families, PCPs, therapy providers, and internal teams to drive optimal outcomes. 

Hybrid Schedule:

  • 60% in-person (minimal in-office time for meetings/training)
  • 40% remote

Key Responsibilities:

Patient Care & Coordination

  • Manage a diverse caseload with a focus on quality care and efficiency.
  • Provide oversight, coaching, and support to CNAs.
  • Develop and implement personalized care plans based on patient goals.
  • Coordinate with PCPs, therapy providers, and support teams for seamless care.
  • Educate and support patients and families on treatment and health management.

Clinical Documentation

  • Maintain accurate, real-time electronic charting.
  • Ensure compliance with regulatory and client standards (24-hour turnaround).

Communication & Collaboration

  • Build strong relationships with PCPs, therapy providers, and internal teams.
  • Escalate concerns to Clinical Leads as needed.
  • Participate in on-call rotation (~one weekend per month), holiday rotation, and possible weeknight phone triage.

Job Requirements

Qualifications:

  • Associate or Bachelor’s Degree in Nursing
  • Active RN license in Colorado or a Compact/Multi-state Unencumbered License
  • Minimum 2 years of clinical RN experience (home health experience preferred)
  • Case management/supervisory and pediatric/adult care experience a plus
  • Familiarity with Medicaid paid family caregiving & CNA scope of practice (training available)
  • Reliable transportation, valid driver’s license, and auto insurance
  • Flexible schedule with local travel and on-call availability
  • OASIS knowledge preferred (training provided)
