Hospice Registered Nurse (RN) Case Manager

WSi Healthcare is seeking a dedicated and compassionate Hospice RN Case Manager to provide high-quality, patient-centered care in Grand Junction, CO. Position Details

Coverage Area: Travel throughout the territory, primarily Mesa County

Schedule: Monday–Friday | 8:00 AM – 5:00 PM

As an RN Case Manager, You Will
-
Assess and monitor patients’ physical, emotional, and psychological needs
-
Develop hospice care plans aligned with patient wishes and goals
-
Provide direct nursing care, including medications, treatments, and interventions
-
Manage pain and symptoms effectively to support patient comfort
-
Educate and support patients’ families and caregivers
-
Collaborate closely with the interdisciplinary care team
-
Maintain accurate and timely documentation
-
Participate in an on-call rotation as required

Job Requirements

Qualifications
-
Graduate of an accredited nursing program
-
Current RN license in the state of operation

Minimum 1 year of nursing experience (3+ years preferred)

Hospice or related experience preferred

Knowledge of hospice philosophy of care

Strong clinical and documentation skills

Valid driver’s license and reliable, insured vehicle
