Key Responsibilities:
- Lead and oversee the daily operations of home health and/or hospice services.
- Provide clinical and non-clinical supervision, maintaining 24-hour accountability for patient care and operational performance.
- Ensure compliance with state, federal, and TJC regulations to deliver high-quality, compassionate care.
- Drive performance improvement initiatives focused on clinical outcomes, financial results, and meeting patient, family, and referral expectations.
- Foster cross-functional collaboration within the organization to create a cohesive and supportive care environment.
- Act as a champion for service excellence, aiming to position the agency as the preferred provider in the community.

Job Requirements
In addition to bringing human kindness to the workplace each day, qualified candidates will need the following:
- Associate Degree Nursing
- Bachelor required within 2 years of hire or transfer for Nurse candidates only.
- 1 year of recent home health/hospice/palliative care. Requires the employee to demonstrate competency in the provision of individualized care of patients based on the assessed age and developmental needs.
- BLS
- Also accept Clinical License as related to degree- RN
- Valid Colorado Driver's License
Medium Work - exert/lift up to 50 lbs. force occasionally, and/or up to 20 lbs. frequently, and/or up to 10 lbs. constantly
