- Collaborate with interdisciplinary teams to develop and execute individualized care plans.
- Provide patient and family education on disease management, medications, and best practices for maintaining health at home.
- Monitor patient conditions, reporting any changes promptly to the care team.
- Adhere to all agency policies, procedures, and regulatory standards.
Preferred Qualifications:
- Familiarity with electronic health record (EHR) systems used in Home Health.
- Excellent time management and organizational skills.

Job Requirements
Requirements:
- Active Registered Nurse (RN) license in the state of Colorado.
- Minimum of 1 year of recent Home Health experience required.
- Proficiency in or experience with OASIS documentation is mandatory.
- Strong clinical assessment and critical thinking skills.
- Valid driver’s license and reliable transportation for travel within the Pueblo area.
- Current BLS certification.
