Registered Nurse (RN) Case Manager – Home Health

WSi Healthcare is seeking a compassionate and experienced Registered Nurse (RN) Case Manager for a full-time home health position in the Colorado Springs area. This hybrid role offers the opportunity to make a meaningful impact by coordinating and delivering high-quality in-home care to patients and their families.

Responsibilities

Manage a diverse caseload and oversee individualized patient care plans

Coordinate care with physicians, therapists, and interdisciplinary support teams

Supervise and support Home Health Aides (HHAs) to maintain clinical quality standards

Conduct patient assessments, provide education, and support patients and families throughout the care process

Maintain accurate, timely documentation in compliance with agency and regulatory standards
-
Participate in an on-call rotation (approximately one weekend per month)

Requirements

Active Registered Nurse (RN) license in the state of Colorado.

2+ years of nursing experience; home health experience strongly preferred.

Case management or supervisory background a plus.

Knowledge of OASIS preferred (training available).

Reliable transportation, valid driver’s license, and current auto insurance required.

Strong communication, organization, and problem-solving skills.
