What You’ll Do:
Provide top-tier nursing care in the comfort of patients' homes.
Complete OASIS assessments and ensure accurate, timely documentation.
Collaborate with an interdisciplinary team to create customized care plans.
Educate patients & caregivers on medications, disease management, and home care best practices.
Monitor patient conditions and report changes to the care team promptly.
What We’re Looking For:
Licensed RN with home health experience (or a strong desire to transition into home health).
Comfort with EHR systems and electronic documentation.
Independent, organized, and able to manage a flexible schedule.

Job Requirements
- Active Registered Nurse Unencumbered License
- Valid driver’s license/vehicle insurance
- Territory: Castle Rock/Woodland Park/Colorado Springs/Pueblo/Canon City
- BLS - AHA
- Covid vaccinated
- Flu season (flu shot)
- Lift 50 lbs
- Clean Background Check
TRAVEL Nurses - Must have 2 or more years of recent experience in ER, ICU, MED SURGE Hospital Units.
