Home Health LPN – Colorado Springs, CO | No Weekends, No On-Call!

What You’ll Do:
Provide hands-on nursing care to patients in the comfort of their own homes
Work closely with a collaborative care team to develop personalized care plans
Educate patients and families on medications, disease processes, and home care routines
Keep a close eye on patient progress and report changes proactively
What We’re Looking For:
Active LPN license
Home health experience of at least 1 year
Solid documentation skills and comfort with EHR systems
Confidence working independently and managing your own schedule

Job Requirements
- Active Registered Nurse Unencumbered License
- Valid driver’s license/vehicle insurance
- Territory: Castle Rock/Woodland Park/Colorado Springs/Pueblo/Canon City
- BLS - AHA
- Covid vaccinated
- Flu season (flu shot)
- Lift 50 lbs
- Clean Background Check
TRAVEL Nurses - Must have 2 or more years of recent experience in ER, ICU, MED SURGE Hospital Units.
