What You’ll Be Doing:

Conduct OASIS assessments and maintain accurate documentation

Collaborate with an expert care team to create and follow personalized plans

Educate patients and families on treatments, meds, and home care

Monitor health changes and communicate updates to the team
What We’re Looking For:

Licensed RN with home health experience

Skilled in Electronic Health Records (EHR)

Job Requirements
- Active Registered Nurse Unencumbered License
- Valid driver’s license/vehicle insurance
- Territory: Castle Rock/Woodland Park/Colorado Springs/Pueblo/Canon City
- BLS - AHA
- Covid vaccinated
- Flu season (flu shot)
- Lift 50 lbs
- Clean Background Check
TRAVEL Nurses - Must have 2 or more years of recent experience in ER, ICU, MED SURGE Hospital Units.
