Case Manager
Job Description
I’m partnering with a great pediatric home health organization that’s expanding their IHSS (In-Home Support Services) program in Colorado and looking for an experienced Case Manager to join their team! This hybrid role offers the chance to make a real difference for children and families providing hands-on supervision, care coordination, and caregiver support to help kids safely thrive at home.
Position Highlights:
- Hybrid schedule
- Oversee pediatric IHSS cases and provide in-home supervision
- Conduct skills validation and competency checks for attendants
- Review and update care plans and documentation
- Investigate and resolve client or caregiver concerns
- Collaborate with families, authorized reps, and care staff

Job Requirements
- Requirements:
- Bachelors degree in Human Services or similar
- 2+ years of nursing experience (pediatric or home health preferred)
- Reliable transportation and valid driver’s license
- Familiarity with Medicaid/IHSS
