RN Case Manager - Temp to Perm Opportunity
Job Description
WSi Healthcare is currently seeking an outstanding Registered Nurse (RN) Case Manager to join a prominent Home Health Care Facility in the Denver area. This position begins as an 18-week Temporary role, transitioning to a Permanent role upon successful completion of the initial period.
Key Duties and Responsibilities:
- Conduct initial and ongoing comprehensive assessments, incorporating the Outcome and Assessment Information Set (OASIS).
- Develop and adjust care plans as necessary to meet patient needs.
- Provide education, supervision, and counseling to clients and their families on disease processes, self-care techniques, preventive strategies, and other relevant issues, promoting client/family independence.
- Deliver specialized nursing services that require considerable skill and expertise.
- Implement appropriate preventive and rehabilitative nursing procedures.
- Supervise and instruct Licensed Practical Nurses (LPNs) and home health aides as required.
- Maintain detailed and accurate records and reports, including those needed by Medicare and other third-party payers.
- Communicate any changes in client needs to the physician and other pertinent personnel.
- Evaluate the effectiveness and outcomes of care provided.
- Plan and coordinate client discharge from services.
- Act as the coordinator of client care, making referrals to other services as needed.
- Participate in in-service programs and maintain continuing education by obtaining 12 Continuing Education Units (CEUs) annually.
- Be accessible to clients and staff by phone and in-person from Monday to Friday, 8 AM to 5 PM.
- If the patient census is low, assist with office duties, including participating in the on-call rotation after 5 PM on weekdays and during weekends, depending on the office location.

Job Requirements
Minimum Qualifications:
- Education: Preferred degree in Nursing from an institution accredited by the National League for Nursing.
- Experience: At least one year of home care experience is desirable.
Knowledge, Skills, and Abilities:
- Comprehensive understanding and proficiency in contemporary nursing practices.
- Familiarity with the team nursing model.
- Competence in developing and contributing to client care plans.
- Capability to accept and utilize supervision effectively.
- Skilled in recognizing individual needs and concerns, and fostering positive working relationships.
- Proficiency in supervisory roles.
- Excellent oral and written communication skills.
Special Requirements:
- Active licensure to practice as a registered nurse.
- Relevant special certifications as required.
- Valid and current state driver's license.
- Up-to-date auto insurance coverage.
- Reliable access to a personal vehicle.
