Pediatric Home Health Case Manager - Registered Nurse

Position Overview:
We're looking for an RN Case Manager who is excited to oversee comprehensive care and coordinate services for patients on their caseload. If you have stellar organizational skills, strong communication abilities, and a passion for high-quality patient care, this role could be perfect for you! You'll be working closely with patients, families, Primary Care Physicians (PCPs), therapy providers, and internal support teams to ensure the best patient outcomes.
Reports To: Clinical Lead
Key Responsibilities
Patient Care and Coordination:
- Manage a diverse caseload, ensuring top-notch patient care and efficient care coordination.
- Provide coaching, oversight, and support to Certified Nursing Assistants (CNAs) to maintain high-quality patient care and stick to clinical standards.
- Develop and implement individualized care plans based on patient assessments and goals.
- Coordinate with PCPs, therapy providers, and internal support teams to offer comprehensive care.
- Educate and support patients and families regarding their healthcare needs and treatment plans.
Clinical Documentation:
- Maintain accurate and timely electronic charting, preferably in real-time.
- Ensure all documentation meets regulatory and client standards within a 24-hour timeframe.
Communication and Collaboration:
- Foster and maintain effective communication with PCPs, therapy providers, and internal support teams.
- Promptly escalate relevant issues to Clinical Leads, such as order management and patient concerns.
- Participate in on-call rotations (approx. one weekend per month), holiday rotations, and possibly phone triage night coverage during weekdays.

Job Requirements
Qualifications:
- Education: Associate Degree in Nursing (ADN) minimum, Bachelor of Science in Nursing (BSN) preferred.
- Licensure: Current Registered Nurse (RN) license in the state of Colorado.
Experience:
- At least 1 year of nursing experience.
- Experience with pediatric care preferred.
- Home health experience (adult or pediatric)
Skills:
- Highly organized, efficient, and independent worker.
- Flexible with scheduling and adaptable to change.
- Strong relationship-building and communication skills.
- Reliable transportation is essential.
- Strong understanding of CNA scope of practice (training will be provided if needed).
